Summer 2014 Twitter Favorites and Parent Questions is a great resources for automating all sorts of actions between your sites, phone, apps, etc. I set it to save my Twitter favorites to a Google Spreadsheet. I just finished going through and adding descriptions to each of my favorites. Unfortunately, I did not find the one thing I was looking for, 7 questions for parents. Thankfully I remembered it was on Edutopia, and it was still listed as trending.

I took the questions for parents and turned it into a Google Form:
